During our meeting we gather information about the person’s:

• Health
  
• Lifestyle
  
• Financial situation
• Family background

The assessment determines the type of home to choose, such as moving in with family, moving to a smaller home, a condominium, apartment or an assisted living residence.

If the decision is to move, Senior Transition will work with the family and the senior to help them find a new home that will provide the comfort and services needed. We will conduct a search with the senior and show them homes that meets their profile.
